STATEMENT

A request for receipt on information from the Register of Natural Persons should be submitted to the OCMA, stating the following information:

  • name(s), surname(s) and personal identity number (in the case of not having a personal identity number, date, country and place of birth or personal identity number included in foreign personal identification document);
  • address of declared or indicated place of residence, contact details (e-mail, indication of an official electronic address (e-Address) or phone number);
  • the amount of information required; (when requesting information about yourself, your minor or a person under guardianship or custody for submission abroad, it is possible to request an EU standard multilingual form to have the statement accepted in an EU Member State without translation).
  • preferred type of receiving the information (depends on the type of request submission, see the table below);
  • the purpose of the use of the information (if the statement is to be used in another country, please indicate the country where the statement is to be submitted);

You can check information about yourself, your child or a person under guardianship or trusteeship in the Register of Natural Persons using the e-service “My data in the Register of Natural Persons “.

Fee

For obtaining a statement from the Register of Natural Persons (also a statement of personal status) about yourself, your child under 18 or a person under guardianship or trusteeship (per person), you must pay a state fee*

Payment of the stamp duty shall be made prior to obtaining information from the Register of Natural Persons according to the type and deadline of the requested statement.

Payment can be made using the e-service “Payment for services provided by the Office of Citizenship and Migration Affairs”

* The amount of the state fee for receiving information from the Register of Natural Persons in accordance with the Cabinet Regulation of 6 July 2021 No. 485 “Regulations Regarding the State Fee for the Receipt of Information from the Register of Natural Persons” in accordance with Paragraphs 2.3., 2.4., 3., 4. and 8 .

In accordance with Regulation (EU) No. 2016/1191 on promoting the free movement of citizens by simplifying the requirements for presenting certain public documents in the European Union, some public documents and certified copies thereof are exempt from legalisation and Apostille certification (certification of authenticity of the document) formality in the EU as of 16 February 2019.

A request for information from the Register of Natural Persons must be submitted to the OCMA, which must include the following information:
  • for a natural person – name(s), surname(s) and personal identity number (in the case of not having a personal identity number, date, country and place of birth or personal identity number included in foreign personal identification document); and for a legal person – name and registration number;
  • natural person – address of declared or indicated place of residence, contact details (e-mail address, e-mail or phone number), legal person – registered office and contact details (e-mail address, e-mail or phone number);
  • the amount of information needed;
  • the reasons for the need to receive the information, accompanied by the document certifying the need or a copy thereof, if any;
  • data from which the person about whom information is requested can be identified in the Register of Natural Persons;
  • the purpose of the request for information;
  • preferred type of receiving the information(depends on the type of request submission, see the table below).

Fee

For obtaining statement from the Register of Natural Persons about another person (about one person), if the information is prepared according to the criteria specified by the information requester, a state fee must be paid*

Payment of the stamp duty shall be made prior to obtaining information from the Register of Natural Persons according to the type and deadline of the requested statement.

* The amount of the state fee for receiving information from the Register of Natural Persons in accordance with the Cabinet Regulation of 6 July 2021 No. 485 “Regulations Regarding the State Fee for the Receipt of Information from the Register of Natural Persons” in accordance with Paragraphs 2.3., 2.4., 3., 4. and 8 .

The owner of the property or his authorised person may request in writing and receive free of charge the current information contained in the Register of Natural Persons on the persons who have declared, registered or given as an additional address their residence in the property, or on the number of such persons (see information)

For sending the certificate by post, a service fee shall be charged in accordance with the Cabinet of Ministers Regulations of 12 September 2023 No. 515, paragraph 12 of the appendix “Price list of fee services of the Citizenship and Migration Affairs Administration” .
